At a guess, the timeline will be something like this:

OpenDOS sources and binaries released. People ditch their pirate
copies of DOS6.0 and use the Caldera binaries.

Hacker dudes d/l the sources and each of them designs a new
extension-API 32 bit DOS by hacking EMM386 and the kernel.

Millions of incompatible variations emerge and swirl around like
things that swirl around.

A few really fast or powerful 32 bit DOS+ architectures emerge, yet
are still basically incompatible. API libraries for DJGPP and other 32
bit compilers are written to access these.

Then, in parallel:

 - A DJGPP API is written that autodetects the installed DOS+ and uses
the appropriate fast 32 bit I/O API, but can only use shared
functionality.

 - A new DOS+ is written that provides all 3 or 4 prevelant APIs.
