Are the sources for xray available?  If so, can you find out what
interrupts does it catch, and what special handling does it do to the
intercepted interrupts?

In any case, you should be able by now to tell what functions of which
interrupts does FORMAT call in the case of write-protected floppy, and
how is that different from a writeable floppy.  Does this info give
any clues?
